In this section of the web site, you can find forms, maps and online services, such as Rec-Connect. Rec-Connect is the City's online system for registering for City recreation classes. This section also includes the easy-to-use Councilmember Locator. By simply typing in your home address, you can learn which member of the City Council represents your ward as well as easily access an e-mail link for that Councilmember.
If you need to find an e-mail address or phone number for a particular City of Plymouth service or program, check out the Contact Us link. It lists information by service and division.
Some of the forms included in this section may be filled out online and submitted via the web site. Others will need to be printed out, completed and mailed back. By providing online forms of both types, we hope to save you a trip or phone call. As it becomes feasible, we will add more forms to the web site. Because most of the forms on the web site are in PDF file format, you will need to have Adobe Acrobat Reader on your computer to view them.
