This section of the web site includes forms. Because most of the forms on the web site are in PDF file format, you will need to have Adobe Acrobat Reader on your computer to view them. In many cases, the link to the form will first take you to background information you may need before completing the form. In those cases, the link to the form will be included in the background information.
Please note that forms on the City of Plymouth web site come in three different types.
- Online Forms - May be filled out and submitted online. To use this type of form, simply type in your answers and click the "Submit" button at the end of the form when you are finished. It will be automatically sent to the appropriate City department.
- Fill and Print Forms - These forms allow you to type in your answers on your computer and then print out the completed form. You may also simply print these forms out and complete them by hand. In all cases, "fill and print" forms will require that you return them via the mail.
- Print Only - These forms can only be printed, completed by hand and returned by mail.
